NSF Mid Point Review - Stakeholder Questionaire

 

We are keen to gain the opinion and support of valued Long Term Conditions stakeholders.

2010 will be five years since the NSF was published. We would like to propose that we undertake a midpoint review to identify progress and ways to reinvigorate this agenda to reach the target of full implementation by 2015.

To help us develop this thinking, we would be very grateful if you could answer a number of questions via this on-line survey.
