This on-line application is used for those who are applying to head coach competitive teams, whether they be returning coaches or new coaches.

A response is mandatory (*) to many questions. If not applicable, please insert a "N/A" in the comment box.

District rules limit the number of teams an individual may serve as the head coach to three (3). LGUSL presently adheres to a policy of two (2)teams.

One head coach application is required per team.

Assistant coach applications are not required.

Please note that information collected in this survey may be used to run a background check on coaches selected by the League. CYSA-North or CCSL will also require your consent to a background check as part of Form 1628 prior to registration.

By submitting this survey, you consent to a background screening report that may include information from public or private sources regarding your character, driving record, criminal history, court records, qualifications and experience, and/or other information relevant to your volunteer service with LGUSL.

In submitting this survey application, you further understand that you are volunteering your services and affirm that you shall not be considered an employee or subcontractor or independent contractor of LGUSL.

6. What gender are you applying to coach?

7. What age group are you applying to coach (for Fall '13)? For example, if you are coaching U8 this season and you want to coach that same age group next year, you'd be applying for U9 next season.

8. What type of team are you applying for? Please note LGUSL expects sufficient turnout to form two (2) BU9 teams. Absent unique circumstances, approval for the formation of a 3rd in a particular age/gender division is rare.
9. Coaching applicants for the newly formed U9 teams are likely to be asked to coach two teams at this age group. If applying for such a coaching appointment, are you willing to coach both teams?
