November 2013 Coaches Challenge Tracking Form

This is the tracking form for new recruits gained November 1st-31st, and is due on Thursday December 6th, 2013.

A new recruit is anyone who has signed up to be a mentor with your program and has expressed a long-term commitment. Because the process to become a mentor can take several weeks, individuals do not necessarily need to be matched during the reporting period they are counted in.

Thank you again for your involvement throughout this year's Coaches Challenge! This is the final tracking form, and this month's drawing will be drawn from all completed tracking forms we have recieved throughout the challenge. We hope you enjoyed the Coaches Challenge as much as we did, and you'll join us again next year!
