Clergy Wellness Program HRA August 2011
1. Introduction
Thank you for agreeing to take part in the Clergy Wellness Program. As part of your participation in this program, you are being asked to complete this Clergy Health Risk Assessment (CHRA) in advance of your participation in this program on August 30-31.
The CHRA is conducted by the Clergy Wellness Program and is being administered by its partners at the University of Kansas School of Medicine-Wichita (KUSM-W) in order to help provide you with a comprehensive health profile for you to use at the 2011 Annual Conference.
The CHRA addresses your holistic health, and, as such, this assessment may take you up to 30-45 minutes to complete. While we recognize this time burden, it is important that the information you provide serves as the foundation for your journey through the Clergy Wellness Project. Results from this baseline assessment as well as two follow-up assessments you be used to help us assess what impact participation in the Clergy Wellness Project will have on your overall health and wellness.
Your participation in the Wellness Project and your completion of the CHRA is completely voluntary and greatly appreciated. You may cease completing the CHRA at any time at no penalty to yourself and you may elect not to answer any question that you are uncomfortable answering at no penalty to yourself. The information you provide as part of this follow-up assessment will only be shared with research personnel at KUSM-W as well as relavent portions of the CHRA with your physician, life coach, spiritual advisor, financial advisor, fitness advisor, and nutritional advisor for use at the retreat. All data acquired from these assessments will only be reported in aggregate with no individual identifiers included.
