Thank you for taking this survey about chlamydia testing. We want to get young people's views on the current programme for chlamydia testing to help us to improve the services available to you.

This first page asks some questions about you - your age, gender and so on. We also ask some questions about whether you've had sex. Your answers are completely anonymous - that means nobody will be able to identify you and we won't tell anyone what you answer.
