Dear Phoenix Businessperson:
We are a team of researchers at Arizona State University who are working with Arizona Public Service and the City of Phoenix to evaluate the Energize Phoenix program. We are interested in understanding the opinions of people who have expressed interest in the Energize Phoenix program, and would like to ask you a few questions about your business and your feelings regarding energy use.
We are inviting your participation, which will involve approximately 20 minutes of your time. Participants must be 18 years old or older to complete the survey, and, if possible, should be the primary APS account holder or one of the business decision makers. Your participation in this study is voluntary. You may skip any questions that you wish, or you may choose not to participate. If you choose to skip questions, or not to participate at all, there is no penalty to you. It will not affect your rights to participate in the Energize Phoenix program, or any other programs sponsored by the City of Phoenix or APS.
The results of this research study may be used in reports, presentations, and publications. Your responses will be completely confidential – your APS account number and/or address will be removed from your responses upon receipt. There are no foreseeable risks or discomforts to you for your participation. Additionally, your completion of this survey in no way obligates you to participate further in the Energize Phoenix program, nor does it obligate you to action of any kind.
The feedback we receive from this study will help us evaluate the effectiveness of the Energize Phoenix program, and how we may better address the interests and concerns of the community when it comes to matters of energy conservation.
If you have any questions or concerns, please feel free to contact Susan Ledlow at (480) 965-8645 (Susan.Ledlow@asu.edu), or Samantha Neufeld at 949-842-3497 (Samantha.Neufeld@asu.edu). If you have any questions about your rights as a participant in this research, or if you feel you have been placed at risk, you can contact the Chair of the Human Subjects Institutional Review Board, through the ASU Office of Research Integrity and Assurance, at (480) 965-6788.
Completion of the questionnaires will be considered your consent to participate. Thank you very much for your valuable input!
Sincerely,
Susan Ledlow, Ph.D.
