WHY ARE WE CONDUCTING A SURVEY?

This is the first Survey that the Sarah Matheson Trust has undertaken and we would be grateful if all members could complete it. The aim is to gain a clearer picture of your experience of life with MSA, whether you have MSA or are a family member and/or carer of someone with MSA.

We would like to find out how easy or difficult it has been for you to obtain information about MSA, how you value the services offered by the Trust, how supported you feel and if there is additional assistance we can provide.

We want to build a complete picture of the requirements of our members, and welcome responses from people with MSA, carers and family.

The results of the Survey will make an important contribution to future plans for building and enhancing our services. They will enable us to provide you with the most relevant and useful information.


HOW DO I COMPLETE THE SURVEY?

This is the online version of the survey. Please complete by MONDAY 28th JUNE 2010. Once you have reached the end, simply press the 'Done' button and we will automatically receive your answers.

IS THE SURVEY CONFIDENTIAL?

All personal details will be treated as confidential and handled in accordance with the Data Protection Act 1998. Contact details are requested in an additional section at the back of the Survey. While this is optional, completion of this section will assist us in maintaining accurate records of our members.

We will publish a summary of the results on our website and in our magazine. All individual responses will remain anonymous.

Please note, we often refer to the Sarah Matheson Trust for Multiple System Atrophy as "the Trust" throughout the survey.
