Welcome to the College Students with Disabilities and Involvement survey.

This survey has a total of twenty-six (26) questions. Some of these questions require you to type a response while others require you to choose from a list of possibilities. Please answer thoroughly using as much detail as possible, which will promote accurate interpretation of your responses. Also, please answer all of the questions with an asterisk (*) to be eligible for participation in this study. Any incomplete surveys will be thrown out.

By continuing with this survey, you agree that I can use the information contained in your responses to this survey in my research paper, which might be shared with campus staff, faculty, organizations, and published in a journal or publication. Identifying characteristics will be excluded for anonymity.
