Manuals for the 2012 Collaborative Summer Library Program (CSLP) summer reading themes will ship in fall 2011. The 2012 themes are "Dream Big - Read!" (children); "Own the Night" (teens); "Between the Covers" (adults).

The purpose of this survey is to collect your preferences for delivery of the 2012 CSLP manuals to your library system.

Before completing the survey, please verify that only one person from your library system is submitting this survey to the State Library.

2. Manuals are typically delivered to the attention of "Youth Services" unless otherwise specified. If you want your manuals delivered to the attention of a specific person, please provide his/her name and title:

3. Does your library system require a DIFFERENT number of manuals than the number you received for 2011? If so, how many manuals do you request? (Maximum request is equal to the number of libraries in your system)

4. The manual is available in two formats:
* looseleaf-binder-ready paper with 3 identical DVDs (this is the format you have received in the past)
* 3 identical DVDs only

The DVDs contain the entire contents of the paper manual (as PDF files), plus all the color artwork, videos, and other extras. They are shipped in 3s so you can distribute them to children's, teen, and adult services if needed. If you don't need the full manual in paper, you may now opt to receive the DVDs only and reduce paper waste.

5. The manual is not available for initial shipping in CD-ROM. However, as in the past, we can supplement your paper manual with the electronic content on CD-ROM, shipped later. This option is for libraries that are UNABLE to use the DVD because of limitations of library computers.
