The Coventry and Warwickshire Living Well with Dementia Partnership is a collection of Local Authorities, NHS, Private and Voluntary Sector organisations dedicated to raising awareness of dementia, improving post diagnosis support for everyone who has a dementia diagnosis and their carers, and improving the quality and access that social care and health practitioners have to important service-related information about dementia.

A big part of this work will be the information portal. The portal will be hosted on the Warwickshire Website and provide a one-stop shop of information on all aspects of dementia, with specialist sections dedicated to Carers, people with dementia, social care and health practitioners.

In order to see where best to focus resources on providing the information that you require, we are trying to get a sense of the information that you need to access regularly to do your job.
