You are receiving this survey because you have been identified as someone whose work experience may fall under the professional category of Community Health Worker (CHW).
The goal of this survey is to promote this important workforce, and to collect information directly from CHWs to ensure their voices are included in state and national programs that impact them. This survey applies to all current and former CHWs in NM. All information will be kept confidential and maintained by the New Mexico Department of Health Office of Community Health Workers.
Information about the survey and the general definition of CHW is provided on the next page.
