2009-2010 Learn and Serve: STARS of Texas Progress Report, Grants Through Region 8 ESC (Due 6/30/10)
1. General Directions
1. The report is due by the end of the day on Wednesday, June 30, 2010. To start the report, click on the "Next" button at the bottom of this page. You will be able to leave and reenter the report where you last left off as long as you are working from the same computer that you started with. The web link will remember where you left off based on the last completed page. As you click on the “Next” button in the report, the page will be automatically saved. You can click on the "Prev" button to go back to your previous entries and make changes. Once you click "Done" at the end of the survey, you will NOT be able to make any more changes to the report.
2. This version of the Learn and Serve America: STARS of Texas Progress Report replaces any hard copies that you may have, which are designed only to help orient you to the questions and format of this report in advance of completing the actual report online.
3. Gather any data and/or other related information prior to starting the report. The program requirements referenced in the report are taken from the document "Learn and Serve America: STARS of Texas District Guidelines," which you should have received from your ESC program coordinator. If you have all your information available, the report should take a maximum of 2 hours.
4. To save a hard copy of your report, you will need either to complete the report first in Word and then cut and past it to SurveyMonkey or to contact Service Learning Texas to request a copy by sending an email to info@servicelearningtexas.org.
