The objective of this survey is to draw to the attention of those responsible authorities the dire need for an urgent re-examination of the existing protocol for the diagnosis and management of the symptoms of hypothyroidism.

If your health has been affected because you continue to suffer the symptoms of hypothyroidism when treated with levothyroxine sodium (T4) only, (e.g. Eltroxin, Synthroid) - please complete the following questionnaire.

SYNOPSIS

• Medical science noted the lack of the universal success of levothyroxine sodium (T4) only therapy over 60 years ago. [1-3]

• Medical science discovered the relevant physiology circa 1970.[4-7]

• Medical practice chooses to ignore all these, and other findings.[8-13]

• Medical practice refuses to recognise medical science and the proper and ethical diagnostics and therapies for the continuing symptoms of hypothyroidism, leaving potentially 100,000 to 1,000,000 undiagnosed and untreated in the UK, and globally, at least five times as many worldwide.[14-27]

1. PLEASE NOTE: This questionnaire is for the use of ONLY those people who were prescribed levothyroxine (T4-only), and who then started using a T3 containing hormone (either synthetic or natural) and their symptoms were alleviated.

Please tick appropriate box ONLY – do not write explanations.

If T4-only (e.g. Levothyroxine/Synthroid/Eltroxin) failed to relieve or eliminate your symptoms of hypothyroidism, did you later have success with a T3 thyroid hormone replacement that did relieve or eliminate your symptoms (e.g. Liothyronine/Cytomel/or Natural Thyroid Extract)?

2. If your doctor, for any reason, stopped prescribing your synthetic or natural form of T3-hormone therapy, and replaced it with T4-only therapy, did this result in a return of your hypothyroidism symptoms?

3. If you were denied a diagnosis because your thyroid and pituitary gland function tests showed a 'normal-within range' result, but you had continuing symptoms of hypothyroidism, were these symptoms relieved or eliminated by you taking some form of a T3-containing replacement?
