This Adoptive Family Questionnaire has been drafted by the Parent Education Subcommittee of the Adoption Community Network. The goal of this questionnaire is to gather information about your interests, needs and desires so that we know what you, as a parent of adopted children, need and want, and can plan programs accordingly.
