This updated Action Plan (2013-2016) intends to build on the initial success of Health Action's Priorities for action and provides an expanded list of evidenced-based recommendations and sub-goals where relevant. Health Action's 2020 Vision and 10 goals were updated to closely align with national, state, and local priorities including the National Prevention Council's National Prevention Strategy, County of Sonoma's Strategic Plan, and Upstream Investments Policy.

Community feedback is an integral part in finalizing Health Action's Action Plan. The following questions solicit feedback relating to the recommendations, current initiatives supporting goals and recommendations, and opportunities for community action in Sonoma County.
1. Recommendations: As mentioned previously, the 10 goals of the Action Plan were developed based on national, state, and local priorities and best practices. The recommendations for action can be found in Figure 3 of the Action Plan.

a. Are there other recommendations that should be considered? Please include citation or link reference documents that provide evidence to support any recommendations that you submit.
2. Current Initiatives: Aligning with existing organizations and initiatives is key to successful implementation of the goals and recommendations. While Health Action has ensured that there is cross-sector, multi-disciplinary community and stakeholder input, there may be more organizations and initiatives that are working towards the same goals and recommendations that are not currently listed. Appendix 3, lists the individuals and organizations that have participated in the development of the Action Plan.

a. Are there existing organizations or initiatives who are working on these goals and recommendations in the county who should be engaged in our collective work? If so, please indicate which goals are being worked on, describe the current activities and provide contact information.
3. Opportunities for Community Action: Health Action wants to continue making progress in all of their 10 goals, but understands the importance of aligning work with existing momentum and opportunities in Sonoma. The Priorities for Action section (page 13) lists three proposed areas of focus for Health Action that aim to make progress towards addressing each of the 10 goals and 24 indicators.

a. Where do you see that Sonoma County has the opportunity to make signficant progress on these goals and indicators? Are there other priority areas that Sonoma County could focus on? (Note: Table 1 of the Action Plan lists indicators that will be used to monitor progress relating to each goal.)

b. Are there special considerations related to these goals for your neighborhood or community that need to be considered and addressed?
4. Is there anything else that you think is important for Health Action to consider?
