2011 Student Stipend Questionnaire

$100 stipends have been made available from the Academy Foundation thanks the generosity of the many Dietetic Practice Groups (DPGs), companies, state affiliates and individual Academy members who supported this program.

Stipend recipients will be selected by random and will be contacted by the Foundation via email in September prior to FNCE.

In order to be eligible for a $100 stipend you must meet the following criteria:

1.) Must be enrolled for Fall 2012 and a student member of the Academy in 2012-2013.

2.) Must be registered for FNCE by September 1st (it is recommended that you register by August 12th in order to receive the reduced early bird registration fee).

3.) Must complete the following brief questionnaire by Thursday, September 1st.

4.) If selected, recipients must be willing to attend the Academy Foundation Student Session, taking place on Sunday, September 25, from 1-2 pm during FNCE.
