UCP Family Survey - 2010

 

Dear UCP Parent or Guardian,

Thank you for your willingness to participate in an important study we are doing at UCP of Central Florida. The purpose of the study is to identify areas that UCP is doing well and which areas we need to improve.

My understanding is that you are a parent or guardian of a child who currently attends UCP of Central Florida. We are asking all parents to share their feedback on areas such as the UCP education and therapy program, the facility and equipment and the support you receive at UCP.

Your responses are completely confidential and will only be released as summaries in which no individual can be identified. This survey is completely voluntary. However, you can help us by taking a few minutes to share your experiences at UCP of Central Florida.
