THE ARC NORTHERN CHESAPEAKE REGION: SURVEY OF FAMILY/CAREGIVER SATISFACTION WITH EMPLOYMENT SERVICES
Purpose and Instructions: The purpose of this survey is to help the Arc Northern Chesapeake Region to determine the quality of its Employment Services operations and how best to improve them. Please complete this survey.

Rating Items: Please rate each item and give each item one rating.

1. The nature and extent to which your family member was actively involved in deciding what type of job s/he would look for?

2. The ability of the Employment Services staff to make use of your family member's interests, preferences, abilities, and needs in helping him/her to decide what type of job to look for?

3. The nature and variety of the employment opportunities that your family member got to explore, or is presently exploring, during the job search and development process (that is, prior to choosing and getting a job)?

4. The ability of the Employment Services staff to make transportation arrangements for your family member and get to and from work?

5. The ability of the Employment Services staff to respond in a timely and effective manner to your family member's concerns about work and/or problems s/he encounters or has encountered in the work place?

6. The opportunities that your family member gets to do things that s/he likes to do during the hours of the day/week when s/he is not working?

7. The ability of Employment Services staff to assist and support your family member in finding a new job in the event s/he became unemployed?

8. The respect shown to your family member by the Employment Services staff?

9. The responsiveness of the Employment Services staff to your calls, concerns, and requests for information?
