This survey is to collect the Summer Reading Statistics for 2011 and the Materials Request for 2012. Please have one person per library complete this form (So, if you have separate children's, teen and adult programs - wait until you have all the statistics and assign one person to complete this form)

You will need the following information to complete this survey:

Library Region, Name, and Telephone Number
Dates of your Summer Reading Program
Name and E-mail addresses of people responsible for the Summer Reading Program(s)

For Children’s Program:
Total number of participants
Number of staff assisting with program
Number of adult volunteers assisting with program
Number of teen volunteers assisting with program
Number of all children’s programs
Total attendance of all children’s programs
Optional: How do you run your Children’s Summer Reading Program?

For Teen Program (if applicable):
Total number of teen participants
Total number of teen programs
