Thank you for your willingness to share your experience with Connecticut services. The Family Support Survey has been completed but please read on about another way to contributer.

The Connecticut Family Support Network, in partnership with the Child Health and Development Institute of Connecticut and the Connecticut Department of Public Health (DPH) is interested in learning about your experiences in obtaining health care for your child with special health care needs. We would appreciate it if you would take the time to fill out the following survey, which should only take a few minutes to complete.
