We want to learn from you! We want to know what you liked about the 200+ HFC activities offered this summer and if there are things you think we can improve. This survey should take 5-7 minutes to complete. ALL SURVEY RESPONSES ARE ANONYMOUS

If you didn't participate in summer activities, we have questions for you too! Complete the first page of the survey and check the box that says you didn't attend any summer activities and a page with questions for you will pop up.

HFC members, a parent(s) and an aide/caregiver should each fill out a separate survey. Each one of you has a different idea of what worked well and what needs tweaking.

PARENTS: Please email this survey to your son/daughter's aide/caregiver so they can add their two cents too. Their caregivers offer a unique perspective and can help us make sure HFC activities are the best they can be.
