Thank you for your participation with SaveNOW a financial education program sponsored by Ohio Treasurer Josh Mandel.

In order to complete the financial literacy portion of your SaveNOW program, you are encouraged to complete the following survey. Your completion of this survey will be used to assess the Treasury's progress towards helping Ohioans better their personal savings rate and develop that savings habit.

Please note-If answering questions for a Custodian/Cosigner account, the custodian should answer the questions as they pertain to the custodian rather than leaving fields blank.
