Survey information
You have been invited to complete this survey because you are a health professional who is likely to have contact with people who have or are at risk of acute rheumatic fever (ARF) or rheumatic heart disease (RHD). Please complete this survey even if you are not familiar with these conditions.
This survey is part of an independent evaluation of RHDAustralia (the organisation coordinating national ARF/RHD prevention and management activities).
It will take about 10 minutes to complete. Responses will not be individually identifiable. The survey will provide RHDAustralia with feedback on the effectiveness of their activities.
Incentive: survey respondents can enter a random draw for two vouchers of $250 and $150.
