ANSWER THESE QUESTIONS FOR YOURSELF. DO NOT BASE YOUR ANSWERS ON WHAT OTHERS MIGHT THINK OR WHAT IS "SUPPOSED" TO BE THE RIGHT ANSWER. THIS IS ALL ABOUT YOU!!!!!!!!!

There will be NO attempt to identify you specifically. Do not write anything but the answer on your survey. All surveys will be collected by students, randomized by students, put in with surveys from other classes and randomized again by students before any of them are tabulated by students and volunteers.

Questions are randomly distributed throughout the survey, so each question is independent of the ones around it. As you answer each question, consider the question on its own. Some questions may seem repetitive, don't worry about that. Please do not go back in the survey and change responses to questions that you have previously answered.

You might find that some of the options provided do not fully represent your opinion. If that is the case, select the one that most closely represents your opinion.

The survey contains a number of very personal questions. Please use the coversheet provided in order to assure the complete privacy of your responses. Please resist the temptation to falsify answers. Accurate answers assure accurate results. This survey is extremely important in determining how WCHS can be better equipped to meet the future needs of the students.
