The objective of this brief survey is to find out how we can improve family support groups so they meet the needs of families. We hope you will take a few minutes to provide some comments or feedback to help us improve the family support group in your community. If you don't have a support group in your community or don't attend the support group please check out the survey anyway and let us know what you need from a family support group.
