Diabetes Survey: Teachers of Grades 7-8 |
1. Introduction
Thank you for responding to our survey about a diabetes curriculum. Please note that we would like you to answer the questions based on what you currently know or think. It is most helpful for us if you do NOT do any research before answering the questions. The questions will ask about your current knowledge of diabetes and about how a unit on diabetes might fit into your curriculum.
The curriculum that we will be developing will focus only on type 2 diabetes (not type 1). Therefore, unless we specify a type, we really mean “type 2 diabetes.”
Finally, the surveys are anonymous. We do not ask for your name or other information that would identify you.
