Introduction
Thank you for participating in this Indiana Standardbred survey. This survey covers operational data for the calendar years of 2007, 2008, and 2009 and questions on the incentive program for 2007 through 2010. Please provide the information below in the spaces provided. In many cases, there may be several potential ways to provide the information requested - please choose what you feel is the most appropriate category when several may seem possible. The most important considerations are that no information is entered twice, and that all requested information end up in one of the categories provided. Blank entries in some categories are acceptable if you feel you do not have relevant information or another category seems more appropriate to you. When in doubt on any item, please take your "best shot" with your response.
In parallel with this effort we'd also like to take the opportunity to update the Standardbred Breed Development Program's contact information. At the same website where you found this survey, please enter your mailing and especially your email address to facilitate future communications. Please note that providing contact information in no way can be tied back to any survey response and is a separate data collection effort.
If you are aware of anyone who did not receive a survey and you think should have, please do not hesitate to make them aware of the website location to find the survey and provide contact information.
Part-owners of horses or operations: If you are part-owner of a horse or operation, please coordinate with your other partners to either submit a single survey response for your entire jointly owned horse(s) or operation(s), or provide your individual responses reflecting your portion of ownership below.
Please provide all financial information in United States dollars (USD). For reference, the Bank of Canada's average exchange rate for the Canadian Dollar (CAD) for 2007 is $1 USD = $1.0748 CAD, for 2008 is $1 USD = $1.0660 CAD, and for 2009 is $1 USD = $1.1420 CAD.
Please enter financial and percentage information as whole positive numbers (no fractions or decimals). Horse counts can be entered as decimals to accommodate part-owners.
Your responses will be strictly confidential. Only summary information across all responses will be shared. No individual responses will be provided to the IHRC or other parties beyond the project team.
Completion suggestions: If you cannot complete this survey in one session, you may stop and return where you left off provided:
1) You have cookies enabled in your browser and cookies are not deleted when you leave your browser. A cookie is only used to save your partial results, and allows you to modify any partial results before you submit using the "done" button on the last page.
2) You use the same computer and same browser (so that the saved cookie is found)
3) You do not hit "done" on the final page until you have completed the entire survey to your satisfaction.
