Welcome to The Ability Center's online intake process for educational advocacy services. Using this form, based on your needs, your child may be assigned an advocate from any one of our offices in Sylvania, Port Clinton, or Defiance.
Please answer the questions as completely as possible. Complete answers help us know how we can best serve your child's needs. If you are a caseworker or other professional helping a family fill out these forms, thank you! Please let us know who you are in the "Who referred you" section of the form.
Once your information is submitted, you will be contacted within two business days so that we can begin working with you.
Our services are free and confidential. Information provided here will be shared with Ability Center staff members who are involved with your case. We will communicate only with the organizations for which you give us permission to do so. You will be asked to provide only the last four digits of your social security number as proof that you are signing the forms electronically.
You are required to complete an electronic signature indicating your understanding and agreement with our Terms of Service and the School District Release of Information before we can work with you beyond an initial consultation. We will never contact a school without having an initial consultation with you or the child's representative first, and verifying that we will be contacting the school.
Releases for other organizations or agencies that you may want us to be in contact with are included. Completion of these is optional.
