The UTMB Health Promotion Program is partnering with Nursing staff to launch a targeted initiative for night shift employees. The program will encompass a variety of worksite programs and services to include; physical activity, enhanced nutritional offerings and education, stress and emotional well-being activities along with other employee requested programs. The following brief survey is provided to gives us an overview of your needs and interests. We look forward to working and interacting with you over the coming months.

1. What shift do you work most often?
2. How many days per week do you engage in moderate to vigorous physical activity, such as: fast walking, jogging, swimming, bike riding for at least 30 minutes per day?
3. How many days per week do you engage in strength training, such as weight lifting or push-ups?
