The goals of this survey are twofold.
#1. First it has been created in an effort to develope a registry of individuals with Functional Needs who may need assistance during a disaster. The information collected will help agencies who respond to emergencies/disasters in determining what Functional Needs exist and what kind of resources we need in our community to improve our response to emergencies/disasters.
This survey SHOULD NOT be considered as an indication that these resources are currently or may be in place now or in the future.
Everyone, with or without special needs, should have disaster/emergency plans and supplies.
#2. Through this survey we also want to develop a group of volunteers who would be willing to help in emergencies/disasters.
This is a voluntary registration. The information you provide will be confidential and will be used by emergency personnel only to determine our community's needs in the event of a disaster.
