Dear HS PAC Mentoring Program's Mentor and Mentee:
Program evaluation is critical to the improvement of the Mentoring Program. The program evaluation forms issued through survey monkey should be filled out by the Mentor and Mentee within six months of the initial Mentor-Mentee match and at 1-year completion of the Mentor-Mentee match.
At this time, we ask that both the Mentor and Mentee fill out the 6-Month mentoring progress survey report. This should take no more than 5 minutes to complete. Your responses will be kept confidential and will be used in the overall evaluation of the Mentoring Program.
