- 2011 990-PF tax return
- The number of grants awarded in your most recent fiscal year
- 2011 or most recent fiscal year investment rate of return and asset allocation
If you do not have all the above figures on hand, please do not let that stop you from participating - there is a valuable portion of the survey you can still complete.
