1. Consent Form
Hi! Thank you for your interest in this study. Just a reminder: this study is for parents and caregivers of children AGES 12-36 MONTHS.
My name is Sabra Melamed and I am a Master's student in the Infant Mental Health program at Mills College. As part of my degree program, I am conducting a study on children's emotional development.
This is an important project because children's emotional development has far-reaching consequences for their success in life. Understanding the factors that promote healthy emotional development will help us nurture positive qualities in the children we work and live with. In addition, this study will provide important information for the families and caregivers of future generations of children. This research is supervised by Dr. Carol George Ph. D., Professor of Psychology at Mills College.
Participation involves you filling out a questionnaire about your child's emotional development. It will also ask you about some other elements of your family life, such as other family members living in your home. Filling out the survey should take between 30-40 minutes.
Participation is completely voluntary. If at any point you do not want to continue, you may stop immediately and there will be no repercussions. In addition, participation is completely anonymous; all families will be assigned numbers and never identified by name, except on the actual consent form. All data will be kept in a locked file cabinet in Dr. George's developmental laboratory office. The only individuals who will see this data will be Dr. George and myself.
I can provide you with the general results of the study upon completion. If you wish to receive a copy of the results, please indicate this on the results form on the last page of the study and provide a mailing address or email address to which they can be sent.
If you have any questions, you may contact me via email at smelamed (@) mills.edu. You may also contact my research supervisor at Mills College, Dr. Carol George, at (510) 430-3268 or via email at george (@) mills.edu.
Please try to answer every question. I deeply thank you for your time.
