1. Introduction

 
Dear Neighbor,

The Richmond District Neighborhood Center invites you to participate in an exciting community needs assessment survey project. This project aims to determine your unique experience, thoughts, and understanding of our programs, services and classes at the Richmond Village Beacon. Through your participation in the attached questionnaire, we hope to ascertain the varied opinions and knowledge base of the adult community members of the Richmond District neighborhood of San Francisco. Your response will help us to learn how to improve existing programs and create new, value added programs to better help, serve and inspire community members.

The survey questions are divided into 3 categories. The first is about "Program Review" and asks you for information about your current experiences with the Richmond Village Beacon and your opinions about existing and new programs, services and classes. The next segment "Visiting the Beacon" asks about your use of the Beacon and the final part is all "About You" where we ask basic demographic questions to help us identify particular characteristics about the community.

Your participation in this study is completely voluntary and all questionnaires will be kept strictly confidential and at NO time will individual questionnaires be released to the general public. To express your views freely inclusion of your name and address is strictly optional.

Attached to this letter is the questionnaire developed especially for you because we believe your opinions will be extremely helpful and very valuable to us. Our questionnaire should take about 10 – 15 minutes to complete, but there is no time constraint. There are also no right or wrong answers.
