It's Crunchtime Pre-Program Survey (Spring 2012)

A lifetime of healthy eating starts at home because parents are their children's best nutrition resource. This program inspires families to make better food choices and rewards kids for eating healthfully at home and at school! Complete this easy, three-minute Pre-Program Survey as part of the It's Crunch Time! Program. Families who complete this survey will receive a large insulated It's Crunch Time! bag (one per family). The Partnership for a Healthier Carroll County will use this survey information to measure the change in eating habits as a result of this program (when compared to data from the Post-Program Survey, which families will complete in November 2012).
