The purpose of this survey is to gain your individual feedback regarding the development of a National Amputee/Disabled Golf Federation Incorporated.

It is a step forward in finding ways for a National body to be able to support all amputees/disabled persons throughout Australia.
This survey is anonymous and no personal details or IP addresses will be collected; it should only take 10 minutes of your time.

1. Should a National Body be formed for Amputee/Disabled golf in Australia?

2. Should there be two representatives from each state association on the board?

3. Should all State Association members vote on the two representatives for their state or should the state association committee decide?

4. Do you think that other non golfing Amputee/Disabled Associations should be allowed to have membership?

6. Would you be happy for the nominated state representatives to begin the formation of the National Body?

7. Should the name of the national association be approved by the state representatives?
