Dear Parents & Players;

Please take a few minutes to complete this survey, which is being sent to you by the four soccer Clubs participating in the West Ottawa Soccer Initiative: Almonte, Goulbourn, Kanata and West Carleton.

This survey asks for your feedback concerning winter development programs that you have been involved in during the past two years. This includes Club-specific programs, and the coordinated winter program offered by the WOS Initiative this past winter 2009/2010. As part of planning the 2010/11 winter program, the four Clubs are evaluating their winter development programs to see what worked, and what could be done better.

In some families, multiple people do winter development. You have the choice of filling in a survey for each player, or of filling in one survey for your whole family, and making comments specific to different players or programs in the spaces provided.

To fill out the survey, complete the questions on each page, and click the NEXT button to move to the next set of questions. If you need to stop the survey before you are done, you may continue the survey later and it will take you to where you left off. If you wish to change your answers, click the PREV button. Once you have completed the survey, please make sure that you click the DONE button. If you are filling in multiple surveys, you MUST complete one before starting the other if you are using the same computer.
