Annually, Reach Out and Read seeks feedback and assessment specifically from our on-site Medical Consultants. Such specific and personal feedback helps us better understand what is (and isn't) working at Reach Out and Read programs across the country. Please answer candidly.

In most cases, our 5-minute survey of 20 questions asks for your own experience, but there are also a few questions targeting the program as a whole. All questions about providers refer to those providers who see children under age six.
