Please evaluate the effectiveness of the East Greenbush Youth Soccer Program by telling us about your child's soccer experience during the Fall, 2011 soccer season as well as the current 2011-12 Winter I & II sessions (for travel players).

Note: If you provide your email address at the end of the survey, you will be entered in a drawing for a $100.00 Price Chopper gift card. Only one winner will be selected. (EGYSO Board members, coaches, and paid staff members are NOT eligible to participate in the drawing).

Please fill out one survey per participating child AND per coach. Example: if your child has or had more than one coach, please fill out two surveys - one for each different coach.
