This form results from research on traits of good teaching practice. All surveys have some pitfalls. To obtain data that will lead to positive change, it is important that the data result from issues rather than general feelings. Try to answer each question specifically without bias that arises from your own general feelings about the instructors.
1. Course Evaluation - Lectures
Below, you will see a series of statements about the instructors and guest lecturers you saw over the past four weeks. Decide how well the statement describes your experiences in this program, and choose a rating between 1 and 5.

Uses a variety of teaching methods besides lectures (demonstrations, field trips, writing, group work, etc.).
Has students apply concepts to demonstrate understanding.
Encourages class discussion / participation.
Is able to accommodate and relate to students as individuals.
Asks questions of students.
Is accessible to students outside of class.
Has genuine interest in students.
Has a concern for the quality of teaching and learning.
Encourages / motivates students to challenge themselves to do high quality work.
Has a sense of humor.
Is enthusiastic.
