Youth Ministry Pre-Survey

I've been working on an in-depth survey concerning our Youth Ministry at Woodway FUMC.
My calling, and my responsibility as the Director of Student Ministries, is to make sure that we create an environment in which each of our member and visiting students have the opportunity to discover and/or develop their relationship with Jesus Christ in a way that authentically connects them and draws them deeper.

I am gathering topics and opinions with this Pre-Survey to make sure I've got everything covered.
I value honest feedback very highly, so don't worry about hurting any feelings.
I also value confidentiality very highly, so NONE of these responses will be shared with anyone else.
