1. Thank you for coming to this page. You might be able to help patients in the Hughenden Valley/Prestwood Surgery areas. You might be surprised at just how much of a positive impact your existing knowledge could help.

Statistics suggest there may be as many as TWO HUNDRED patients in our area who have dementia. This number will definitely rise every year from now on. Here in the Chilterns, we have an older population (relatively speaking) and so Dementia is a more common issue in our area than elsewhere.

At Dr Mitchell and Partners, we are putting together an improved, up to date resource pack for our patients with dementia and their carers.

We want to know what LOCAL services are available for people so that we can provide patients with not only the well-kown helplines and charity information but also things unique and nearby to their own homes.

We have already started this but we know it is our patients who are the local experts on certain services and we know that, collectively, your knowledge surpasses our own.

By telling us of the local, maybe personal, services you know might help our patients who have dementia, you will be directly contributing and improving what care we can provide.

For example, do you know of:
Someone who might be willing to help with collecting prescriptions from the pharmacy?
Someone who can help drive someone to hospital/GP appointments?
Someone who can do the shopping for someone down their street?
Organisations that can help with continence care?
Organisations that might help with foot care and other issues of personal hygiene?
Someone who can help with laundry?
Someone who can help with pet care or gardening?
Home care services that you have had experience with?
