About the Survey
As The Forum Task Force, we are attempting to understand how to collaborate in best serving the needs of the Christian and broader communities in effectively addressing the issues of Sexual Addiction – Prevention, Education, Recovery and Restoration for individuals, marriages and families. Your feedback and input on this survey will assist us in this effort, so your responses are greatly valued and appreciated.
