Background and Purpose of Survey

This survey is being sent to you because your organization helps people in Washington understand public or private health insurance coverage options and may also help people to obtain coverage. The purpose of the survey is to collect information about how your organization provides such assistance. Your organization may use websites and general outreach. However, you’ll see that the focus of the survey questions is on personal assistance, in-person or by phone.

CHOICE Regional Health Network, a nonprofit community organization, is performing this survey for the Washington Health Care Authority (HCA). CHOICE’s assignment for HCA, including this survey, is to provide background on current sources of consumer assistance in support of obtaining health coverage. This will help HCA assess the current state of assistance for health care consumers in Washington State as a part of planning the Health Benefit Exchange. The HCA assessment will fulfill a federal grant obligation. More information on the planning of the Health Benefit Exchange is available at http://www.hca.wa.gov/hcr/exchange.html.

This is not a scientific survey and it will not be interpreted in a highly quantitative manner. CHOICE is using the survey as an efficient way to learn more about the service you, and similar organizations, provide. The survey, and CHOICE’s report to HCA, will not support or promote any type of service or consumer assistance. The report will be far too generic to do that. Your answers will provide CHOICE with background information about the variety of consumer assistance now available to people looking into or signing up for coverage in Washington State.

Please take the time to complete this survey and return as soon as possible, but if at all possible by December 9. The survey should take about 10-15 minutes.

Feel free to omit any answers you wish, while understanding this will provide us a less complete sense of existing Consumer Assistance.
