To be considered for a tree you must click below:   yes -- I hereby understand and agree that, by signing and submitting this card, I am authorizing the installation of a shade tree in my front yard IF my property is selected to receive such a tree. My authorization includes the right of the City or its contractor(s), without any further notification, to come on my property for utility locations, tree siting, tree hole digging, and planting of a tree chosen by the City. If it should be determined that there are unsatisfactory soil, environmental, or other adverse conditions on my property, the City or its contractor(s) shall have the right not to plant a tree but will restore any disturbed portion of my property to approximately its same condition prior to the discovery of the unsatisfactory condition. The City and its contractor(s) make no warranties or representations about the health or condition of the planted tree or its likelihood to thrive after planting, although a property owner is encouraged to contact the City if the tree appears to be unhealthy. I understand I may withdraw the authorization set out above if I contact the City at 720-913-0681 prior to any on-site work by the City or its contractor(s). Once the tree is planted, I agree to care for and water the tree.
