Official 2012 UNA-USA Annual Report

 
Each Chapter is required to submit one annual report per their Chapter affiliation agreements. The report consists of questions on your chapter, membership, and advocacy; it should take approximately 30 minutes to complete. You will be asked to submit the names of your current officers, basic financial information, and an overview of your Chapter’s activity in 2012. Please note that you will not be able to save your work.

Before submitting your report, please print a copy for your reference.
