The purpose of this survey is to measure parent satisfaction with the Math and Science Academy school climate and school curriculum.

We recognize that many factors beyond our control may influence parent satisfaction with MSA, including but not limited to transportation, school start and finish times, and number of mandatory school days. However, because we have no way of changing these items, we will not address them in the survey.

Parents or guardians (not students) should complete the survey. Parents or guardians may complete one survey per household, or each parent or guardian may complete a separate survey.

Please answer each question. Although the survey is comprehensive, it may not cover everything. You will have an opportunity at the end of the survey to comment. The survey will take about 20 minutes to complete. The bar at the bottom of the page will show your progress.

Your survey answers are important as well as anonymous and confidential.
