Dear Alternative-Media User,
You're invited to take this survey about why you like alternative media and how you use it. As a participant, you'll be asked to complete this online questionnaire, which takes about 12 minutes. If you click on the button below, it means that you have read this message and agreed to volunteer.
Hopefully you'll enjoy sharing your ideas about alternative and mainstream media. While there is no direct benefit to you for participating, the results may provide valuable information to the field of mass communication and journalism. As an extra incentive, the first 100 people to complete the survey can enter a drawing for a $100 gift card for Amazon.com; instructions will be provided at the end of the survey.
Your identity as a participant will remain confidential. Your name will not be included in any forms, questionnaires, etc. Any identifying information will be stored securely in a database available only to the investigator and research assistants. Data collected may be destroyed at the end of a legally prescribed period of time or stored for further research. Results will be reported only in the aggregate. If you're interested in seeing the study results, please e-mail your name and address to email@example.com.
The survey is being conducted by Jennifer Rauch, a journalism professor at Long Island University. If you have questions about the research, you may contact either me at 718-488-1535 or my department head, Ralph Engelman, at 718-488-1153. If you have questions about your rights as a subject, contact the executive secretary of LIU's Institutional Review Board, Kathryn Rockett, at 516-299-2523. Your participation is voluntary and will not involve any risk or discomforts. A decision not to participate will involve no penalty or loss of benefits to which you are otherwise entitled, and you may discontinue participation at any time without penalty or loss of benefits to which you are otherwise entitled.
Thanks for your interest in this project!
