In order to receive continuing pharmacy education (CPE) credit for attending "19th Annual Fall Preceptor Symposium", you must complete the activity evaluation and verification of completion form.

The evaluation will take about 15 minutes to complete. At the end of the evaluation you will be redirected to the Verification of Completion webpage. It is mandatory that this form be submitted or a Statement of Credit cannot be issued. The form is in no way connected to your evaluation submission.
