1. INFORMED CONSENT FOR PARTICIPANTS

RESEARCH PROCEDURES
This research is being conducted to assess the library instruction needs of students and faculty at George Mason University’s Arlington Campus Library. If you agree to participate, you will be asked to complete a one-time, confidential survey about your research interests and skills. This survey will require 5 to 10 minutes to complete.

RISKS
There are no foreseeable risks for participating in this research.

CONFIDENTIALITY
The data in this study will be confidential. The survey does not include any information that will directly identify you as an individual. While it is understood that no computer transmission can be perfectly secure, reasonable efforts will be made to protect the confidentiality of your transmission. No identifying information, such as name or computer IP address, will be recorded.

PARTICIPATION
Your participation is entirely voluntary, and you may stop the survey at any time and for any reason. There are no costs to your or any other party. If you decide not to participate or if you withdraw from the survey, there is no penalty.
